The dosage of thiopental as pharmacological cerebral protection during non-shunt carotid endarterectomy: A retrospective study

نویسندگان

چکیده

Background: Thiopental has been used as a pharmacological cerebral protection strategy during carotid endarterectomy surgeries. However, the optimal dosage required to induce burst suppression on electroencephalogram (EEG) remains unknown. This retrospective study aimed determine of thiopental non-shunt endarterectomy. Methods: The Neurological Institute Thailand Review Board approved study. Data were collected from 2009 2019 for all patients who received and had intraoperative EEG monitoring. Demographic information, stenosis severity, parameters, dosage, clamp time, events, patient outcomes abstracted. Results: included 57 patients. Among them, 24 (42%) achieved pattern with 26.3+10.1 mg/kg/hr. There no significant differences in perioperative events between those did not. After surgery, 33.3% extubated awakened. One non-burst group experienced mild neurological deficits. No deaths occurred within one month postoperative. Conclusions: achieve was mg/kg/hr.

Cerebral Protection During Carotid Endarterectomy • Carotid endarterectomy was performed on 73 occasions using the following regimen during the operative period: (1) general anesthesia with normothermia, (2) maintaining a normal or slightly elevated arterial oxygen tension (Pao,, (3) maintaining a normal or low arterial carbon dioxide tension (Pco,), (4) maintaining systemic arterial pressure (...

Cerebral blood flow measurement as a safeguard during carotid endarterectomy.

Cerebral Blood Flow Measurement as a Safeguard During Carotid Endarterectomy • Intraoperative measurement of regional cerebral blood flow (rCBF) by the Xe intracarotid injection technique was performed in 17 patients operated on for stenosis of the internal carotid artery. Mean rCBF decreased from 51 ml/100 gm x minute before endarterectomy to 30 ml/100 gm x minute when the internal carotid art...

Surgical outcome of the non-shunt carotid endarterectomy.

BACKGROUND AND OBJECTIVE Nearly 60 years after the advent of carotid endarterectomy (CEA), controversy continues regarding the necessity of an indwelling shunt during carotid reconstruction. Reasonable arguments have been made for monitoring-dependent shunting, and remarks can be made in favor of non-shunt procedure. Based on the results of several studies, most strokes after CEA are thromboemb...

Cerebral blood flow during carotid endarterectomy.

Cerebral blood flow (CBF) was measured during 28 carotid endarterectomies in 25 patients, by injection of 133Xe into the exposed internal carotid artery (ICA). Twenty-three patients had episodic cerebral or retinal ischemia and five had neurologic deficits before operation. Initial CBF values were variable. Increases of PaC02 caused CBF to increase in four of six patients, to increase slightly ...
